TO THE CONTRACT EFFECTIVE OCTOBER 1, 2023
BETWEEN COUNTY OF FRANKLIN, PENNSYLVANIA
("COUNTY") AND CHAMBERS Rx, LLC t/d/b/a
CHAMBERS' APOTHECARY, ("PROVIDER")
FOR: SUBSTANCE USE DISORDER TREATMENT
THIS AMENDMENT ("Amendment") to the Contract dated October 1, 2023 ("Original
Contract"), is by and between the County of Franklin ("County") and Chambers Rx, LLC,
t/d/b/a Chambers' Apothecary ("Provider"). County and Provider may be individually
referred to as "Party" or collectively as the "Parties" in recognition of the following:
WHEREAS, the Parties entered in the Original Contract whereby the Provider
provides substance use disorder treatment for its Good Wolf Treatment Court (GWTC), and
NOW, THEREFORE, in consideration of the mutual promises contained herein and
other good and valuable consideration, the receipt and sufficiency of which is expressly
acknowledged, the Parties agree to the following changes to the Original Contract:
1. The ending date of the Original Contract, and as previously amended, is extended from
September 30, 2026, to September 30, 2027.
2. Amendment to include mental health and substance use disorder treatment.
3. All other terms and conditions of the Original Contract, and as previously
amended, except as expressly amended herein, shall remain in full force and effect.
To the extent this Amendment conflicts with the Original Contract, the language of
this Amendment shall be binding.
