Provide an overview of a recent program that your organization has led*
Provide an overview of a current or recent program that your organization has led. Include your evaluation of
outcomes/indicator data (results) to affirm your efforts.
The Franklin County Area Agency on Aging (FCAAA) has successfully led many programs throughout the
County. The FCAAA is committed to serving the older adult population, despite funding struggles. One
program the FCAAA currently leads is the OPTIONS in-home care program. Through this program, care
management is available for individuals 60+ who do not qualify for Medicaid programs and who do not have
the resources to pay privately for help at home. There is a sliding copay scale based on an older adult’s assets
and income. Services may include: personal care, adult day care and/or emergency response units. This
program successfully provides options for an older adult to remain safely in their home with additional
supports. Each month, approximately 415 seniors are served through this program.

Senior Transportation
What is the specific need/problem you want to address?*
Describe the evidence of this need/problem by providing data and exact detailed information to support this
statement.
Transportation is a barrier for many older adults to make it to medical, oftentimes even life-sustaining
appointments. Each year millions of older adults have difficulty securing transportation when they no longer
are able to drive. Additionally, approximately 25% of seniors begin skipping medical appointments and lack
of reliable transportation is cited as the number one reason. Franklin County Area Agency on Aging (FCAAA)
is committed to ensuring our seniors have reliable transportation for medical appointments.

your project as if the person reading this does not know anything about your organization and this project.
This program will provide transportation for seniors to go to and from medical appointments, grocery stores,
senior centers, adult day center and work. Due to seniors living on a fixed income and a lack of personal
transportation, many local seniors rely on public transportation to make it to appointments, including
medical appointments. This program covers all fees for seniors, so costs don’t create a barrier. Many seniors
rely on this program for life-sustaining appointments such as dialysis and chemo treatments. For seniors to
qualify for the program, they call the Aging office. An aging care manager completes a screening with them to
assess for financial need. If they meet the financial requirements, we supply their information to our local
transportation provider, Rabbittransit. Rabbittransit then invoices the Aging office for that senior’s trips on a
monthly basis.

Logic Model
Use the Logic Model Template below as a guide for responding to the following questions as the questions are the
same as in the template, just listed.
Problem/Need Statement*
This is a concise description of the problem your project is trying to address.
Many older adults lack affordable transportation, limiting access to medical care, food, and community
services. Seniors needing treatments like dialysis or chemotherapy face serious risks when rides are
unaffordable. The Aging office screens for financial need and covers transit costs through Rabbittransit, but
rising demand requires more funding to ensure all eligible seniors receive reliable, no-cost transportation.

Goals of Program / Project*
What is the main goal(s) of your project? What are you trying to achieve?
The main goal of this project is to ensure that financially vulnerable seniors have reliable, no cost
transportation to essential services that support their health, independence, and overall well-being.
What resources do you have available to work with?*
List your top resources to work with in accordance to this project/program.
These resources could be other grants and programs, existing programs within your organization, etc.
If you plan to collaborate with other groups or organizations list their name and explain in detail how you plan to
do so.
Our top resources for this program include: a contract with our local transportation provider, access to
seniors, care managers with ability to assess financial need and a team to implement this project and collect
data.
What activities will achieve the desired outcomes of this program/project?*
Provide a brief overview of the activities that will occur in your project. You may list these activities. They are what
you will use as your activity #1, #2, #3, etc.
The primary activity is to provide fully subsidized, reliable transportation for financially eligible seniors
through a partnership with Rabbittransit. The Aging office conducts intake screenings to assess need and
eligibility. Approved seniors are enrolled, and their information is shared with Rabbittransit, which provides
trips to essential destinations such as medical appointments, dialysis, chemotherapy, grocery stores, senior
centers, adult day programs, and workplaces. Monthly invoices are sent to the Aging office for completed
rides, ensuring no senior pays transportation costs. This coordinated process removes financial and logistical
barriers so seniors can consistently access the services that support their health, independence, and quality of
life.
ACTIVITY #1
Activity #1 Description*
What are you going to do? Describe, in detail, the main activity that will occur to address the problem need and
reach the program/project goals.
The primary activity of this project is to provide fully subsidized, reliable transportation services for
financially eligible seniors through a structured partnership with Rabbittransit. To carry out this activity, the
Aging office will conduct intake screenings for each senior who requests assistance, during which an aging
care manager assesses financial need and eligibility. Once a senior is approved, the Aging office enrolls them
into the transportation program and communicates their information to Rabbittransit. Rabbittransit then
provides transportation to and from essential destinations, including medical appointments, dialysis and
chemotherapy treatments, grocery stores, senior centers, adult day programs, and workplaces. Rabbittransit
invoices the Aging office on a monthly basis for each enrolled senior’s completed trips, ensuring that no
senior incurs any transportation cost. Through this coordinated process, the program removes financial and

well-being. Access to grocery stores and essential
errands will support improved nutrition, medication management, and daily living stability. Additionally,
transportation to workplaces will help seniors maintain income and independence. Collectively, these
improvements will allow seniors to remain safely in their homes longer, reduce preventable hospitalizations,
and enhance their overall sense of dignity, security, and connection within the community.

EXTERNAL FACTORS
Melodie Hoff Franklin County
Printed On: 12 October 2026 2027 Summit Endowment Level I - Cycle A 10
What barriers could affect your work?*
What are your largest obstacles or barriers that could have an influence on your work that may be out of your
control?
Explain in detail what these barriers are, what impact they could have on your work, and how you plan to adjust
accordingly to them if able.
Several external obstacles may affect the successful delivery of the senior transportation program, even
though they are outside of our direct control. One significant barrier is the availability and capacity of public
transportation services provided by Rabbittransit. Limited drivers, service interruptions, staffing shortages,
or changes in Rabbittransit’s operating schedule could reduce the number of rides available to seniors or
cause delays in getting them to medical appointments and essential services. Such disruptions could lead to
missed or postponed medical treatments, increased stress for seniors, and reduced program effectiveness. If
these issues arise, the Aging office will work closely with Rabbittransit to adjust schedules, prioritize
medically essential trips, and explore supplemental transportation options through volunteer networks or
other community partners when possible.
A second major barrier is fluctuating financial resources. Increases in transportation costs or decreases in
available funding could limit the number of seniors the program is able to serve. Rising fuel prices, insurance
costs, or changes in reimbursement structures may reduce program capacity. If this occurs, the Aging office
will monitor spending closely, seek additional grant funding, adjust financial eligibility thresholds as needed,
and advocate for sustained investment to ensure continuity of services for the most vulnerable seniors.
Finally, broader external factors—such as severe weather, regional emergencies, or roadway disruptions—
may temporarily impact transportation availability. These events can prevent seniors from attending
appointments or accessing necessities. In these circumstances, the program will communicate immediately
with affected seniors, reschedule appointments where possible, and coordinate with community agencies to
provide emergency supports until transportation services resume.
By anticipating these barriers, maintaining strong partnerships, and planning adaptive strategies, the
program will remain resilient and continue meeting the critical transportation needs of local seniors.
ASSUMPTIONS
List any facts or conditions you assume to be true*
List any facts or conditions you assume to be true about your program/project and activities to be performed. This
could also include outcomes that you believe will come from your project but not something that could be easily
measured.
Ex. People will want to sign up for a free Zumba class being offered since they see lots of advertisements about it,
want to lose weight, and it is free.
Facts we assume to be true, based on our knowledge and data with seniors in Franklin County are that
seniors experience transportation barriers due to the costs of public transportation. We are also assuming
that the majority of seniors are willing to take public transportation.

Budget
Use the Budget Worksheet Template Chart below as a guide for responding to the following questions. The
questions below follow the template and are asked in order as if you were filing out each row at a time.
How will you obtain the rest of the funds needed to implement this
program/project?
If this grant will fully fund this program/project, then you can leave it blank.
The program’s operating budget does not fully support the total cost of services. To secure remaining funds,
we will pursue several options. These include evaluating the flexibility within our Human Services Block
Grant, evaluating reserve balances and long-term need, and if necessary, requesting supplemental support
from the County General Funds. Use of reserves will be approached with caution to ensure long-term fiscal
stability.
How will you sustain your program/project after this grant funding ends?*
Ex. work it into the yearly budget, receive funding from another source, or N/A because this is a one time
project/program
Because the services provided through this program are mandated, the County is committed to sustaining
them beyond the grant period. We will continue to explore strategies outlined above. These approaches will
help us integrate the program’s costs into our long-term budgeting process and maintain continuity of
services.
