HomeMy WebLinkAboutMH-204 Long-Term Inpatient Referral1 FRANKLIN/FULTON COUNTY MENTAL HEALTH/INTELLECTUAL & DEVELOPMENTAL DISABILITIES/EARLY INTERVENTION 425 Franklin Farm Lane Chambersburg, PA 17202 (717) 264-5387 MH/IDD/EI PROCEDURE STATEMENT PROCEDURE SUBJECT: Long-Term Inpatient Referral PROCEDURE NUMBER: MH-204 Effective Date: January 1, 2008 Date Revised: October 13, 2022 BACKGROUND: The Danville State Hospital is the designated facility for Franklin/Fulton County individuals in need of long-term inpatient psychiatric treatment. In order to facilitate cooperative service planning between the county MH/IDD/EI Office and the State hospital staff, a case manager through Service Access and Management, Inc. (SAM) has been assigned to function as the liaison between the state hospital and the community Mental Health system. This individual visits the State hospital on an as needed basis. The purpose of these visits could include the following activities: attend commitment hearings, meet with individuals, and, meet with hospital staff to discuss discharge planning. The case manager has contact with each individual and/or their assigned social worker at least quarterly. In Franklin/Fulton County, the “gatekeeper” for Danville State Hospital (DSH) is a Mental Health Program Specialist. Current bed capacity at Danville State Hospital is five (5) beds. The designated SAM case manager who functions as the DSH liaison is kept informed of the status of all referrals. However, an individual who is receiving Blended Case Management (BCM) at the time of commitment to DSH, case management services would be transferred to the DSH liaison (Administrative Case Management [ACM] or Utilization Management level of case management) upon admission to the State hospital. A referral packet that can be used with DSH has been created and distributed to all applicable facilities for their use. 2 PROCEDURE: Community Inpatient Psychiatric Treatment For an individual with PerformCare, the PerformCare Clinical Care Manager (CCM) approves the initial admission and has subsequent continued stay reviews at regular intervals. If it is determined that the individual might benefit from longer term inpatient care, the Mental Health (MH) Program Specialist will receive a completed state hospital referral packet from the referral source (inpatient unit staff). MH Program Specialist will review the referral packet to determine, among other purposes, if the person is currently open with SAM. If not, MH Program Specialist will enter the pertinent information into CPR-web in order to have a Base Service Unit (BSU) number assigned and communicated to DSH personnel. The MH Program Specialist will also convey to DSH personnel whether the referral has been approved at the County level. MH Program Specialist will add the person’s name and other relevant information onto the internal tracking sheet and will have periodic contact with the Admissions Officer at the state hospital to ascertain status of the clinical review process. DSH personnel will inform the MH Program Specialist when a determination has been made regarding the referral packet. MH Program Specialist will continue providing the SAM DSH liaison with status updates including when the person has been approved for admission. Waiting Lists Due to capacity issues, the likelihood exists that occasionally the referred individual’s name will need to be placed on a waiting list for DSH. Discharge Planning As soon as it is determined (a minimum of 30 days prior to discharge) that an individual is ready for discharge from DSH back to the community, the SAM DSH liaison will become actively involved in service planning. Commitment Hearings Hospital personnel will assume lead responsibility for coordinating all commitment hearings and proper and timely handling of related documentation. This applies to all hearings to commit someone to DSH. Facility staff coordinates the hearings during the actual treatment stay. MH/IDD/EI is billed for the Mental Health Review Officer’s (MHRO) involvement in these hearings. County Transfers If an individual, within the first six (6) to twelve (12) months after transferring to a county other than Franklin or Fulton, requires long-term inpatient psychiatric care, that person would be referred/admitted to DSH in a Franklin/Fulton County bed. However, after that time the person would be admitted to a bed of the “new” county of residence. The details of this, including the specified time frame, would all be addressed by a Letter of Agreement which would be prepared by the designated MH Program Specialist and signed by the MH/IDD/EI Administrators of the respective counties.
