MEETING MINUTES Welcome & Introductions This meeting was held via Google Meet. Those present introduced themselves and their agency affiliations during the Provider Updates. Administrative Entity (AE) Updates - ±¨®­ 2®¶¤ %­³¤±¯±¨²¤ )­¢¨£¤­³ - ­ ¦¤¬¤­³ Ȩ%)-ȩ 5¯£ ³¤² Marion ha Developmental Programs (ODP) in a few months. This is a good thing! Providers are doing a great job in getting the reports entered as needed. One of the things Marion has been noticing with incident reports related to a behavioral health crisis whether its facility or community-based is that the information regarding whether was being followed and what staff did to de-escalate the situation is not included in the first section of the report. Please remind your point staff to include this information in the first section of those reports. It will save that feedback from the county and regional initial review. ( ­£®´³² ¥®± ³§¤ -¤¤³¨­¦ - o Marion included handouts for the HCQU trainings that went out with the agenda. This information will also go out with the minutes. IDD Provider Meeting Friday, November 15, 2024 o Marion included communication from ODP regarding death reports. If you are unable to get a death certificate, please make sure you note in the actual report your attempt to get the certificate from a family member. We must make at least one attempt. However, if you are unable to get it, you do not need to keep pursuing that information. You will still need to include the supporting documents uploaded into the incident report before you do finalization (as opposed to sending them to the county office). The supporting documents never change. This is an ongoing requirement for many years. o We received some updated brochures from Cornerstone who provides behavioral supports. These will be sent with the minutes when they go out. 2¤¡¤¢¢  7¤ µ¤± %¬¤±¦¤­¢¸ 0 ¢ª² - Becky reached out to providers to inform them that the IDD program had some additional funds available. The Supports Coordination Organization (SCO) was providing some emergency preparedness kits to individuals. In a way to mitigate some risks, we were looking if we could provide an emergency car kit to include jumper cables, small first aid kit and a car charger as a supplement to that. We are looking to see what funds could be available to provide those to agency vehicles. As of right now, Becky has heard from a various providers which totals 100 agency vehicles. Becky requested that providers email her by end of business today regarding their agency vehicle count. The details pertaining to ordering and pick up are still being arranged. 0±¤²¤­³ ³¨®­ȝ4± ¨­¨­¦ - th o Monday, November 18 at 1 pm is the Social Security virtual presentation. nd o Monday, December 2 at 1 pm is the Power of Attorney/Guardianship virtual training. Ed Wenger will go over the recent and/or ongoing guardianship changes. If you did not receive invites/links for either of these, please email Becky at rmweaver@franklincountypa.gov. *®§­ '¤± ª John does the quality side of incident management. He wanted to remind providers that if there are any extensions that are needed to please get them in on a timely manner so we are not getting any reports back that they are late. If you are having any issues reaching out to the Help Desk do not hesitate to get in contact with the Administrative Entity (AE) staff so they can assist and reach out to ODP contacts. For Quality Assessment & Improvement (QA&I), we are doing cycle two (2), year three (3). John will be finishing up the onsite visits by the end of year. After January, anyone who is scheduled for provider risks, John will be reaching out to schedule the information pieces. IDD Provider Meeting Friday, November 15, 2024 Supports Coordination Organization (SCO) Updates 3¤±µ¨¢¤ !¢¢¤²² lj - ­ ¦¤¬¤­³Ǿ )­¢ȁ Ȩ3!-ȩ As of this week, SAM is officially fully staffed. All twelve (12) Supports Coordination positions are filled. The newest SC is named Rebecca who is currently in the training process. She will then shadow for a few weeks. Shanna is holding off on redistributing caseloads for the moment. If anyone needs any caseload clarification, please reach out to Shanna Golden or Matt Yingling. Matt noted that they are working on getting all the criticals completed for the billing period up to the end of September. If anyone is waiting for any criticals or revisions to Individual Support Plans (ISPs) please make sure you reach out to the SC and include their supervisor in the email so they can help with making sure they get processed quickly. %±¨¤ #®´­³¸ # ±¤ - ­ ¦¤¬¤­³Ǿ )­¢ȁ Ȩ%##-ȩ ECCM was not in attendance. Becky reported that ECCM is now supporting 17 individuals for Franklin/Fulton and has three (3) SCs. Healthcare Quality Unit (HCQU) Updates Lisa Miranda attended from the HCQU. She has the syllabus for this month and next month. There are two (2) trainings left this left 1) Grief and Bereavement and 2) Delusional Disorder. In December, there is 1) Emotional Intelligence which is a newer training, 2) Healthy Nutrition and Weight Management, 3) Universal Precautions and 4) Trauma Informed Care. For syllabus trainings, it is helpful to pre-register but you do not have to. You can just join the meeting. However, you would need to get the link from Katie. Lisa has been running incident reports. She looks for chronic reoccurring illnesses and falls. There is a huge amount of falls that have been happening. The HCQU is still doing the fall risk data collection. A pharmacy review is done with this. The HCQU collects information from the providers. The providers send the HCQU some documents then they send out recommendations based on the information received and what happened with the falls. The recommendations could be taken to the doctor to request a home assessment. If the HCQU is seeing reoccurring concerns related to illness and falls, they reach out directly to the SC. The SC will then reach out to the provider to see if they are interested in that service or if they are interested in training. Marion requested that Lisa include the AE in these emails as they do have provider check in meetings with residential providers. AE staff can then follow up with the providers about the education and support as well as any concerns related to their overall health and wellness. Lisa also looks at death reports and is able to provide grief and support training. Provider Updates &´«« #¨±¢«¤ Full Circle is still looking for residential and Community Participation Supports (CPS) referrals. 4§¤ !2# ®¥ &± ­ª«¨­  ­£ &´«³®­ #®´­³¨¤² th December 12 at Grand Point Church from 6:30 p.m. to 8 p.m. This will be the last event for the year. IDD Provider Meeting Friday, November 15, 2024 3 ¥¤ ¨­ (®¬¤ There was some confusion with some other SCs in regards to Safe in Hom medication pill boxes based on assistive technology versus remote support being covered. Tara Harmening sent an email out stating that they have a new smart pill dispenser that is connected to remote support so they can still do medication management covered through the waiver with the pill dispenser. It sends a text message to remote support if someone chooses to take their medication or miss their medication. A text can also be sent out to the support circle. If anyone has any questions regarding remote supports or if you want a refresher with the SCO or provider, please email Tara. When we send out the minutes, we will also have the provider contact list attached. Please let us know if there are any updates or changes needed to the contact list. This list is available for all providers to be able to contact/connect with other providers. Future Topics If there are any topics that providers want the AE to review, discuss or have a presentation on at a future meeting, please email any of the AE staff so they can look into this.
