AMENDMENT NO. 1
TO THE CONTRACT EFFECTIVE JUNE 4, 2025
BETWEEN COUNTY OF FRANKLIN
AND
SOUTH CENTRAL COMMUNITY ACTION PROGRAMS, INC.
FOR COORDINATION OF EMERGENCY SOLUTIONS GRANT PROGRAM
COMPONENTS
THIS AMENDMENT (“Amendment”) to the Contract for Emergency Solutions Grant
program components dated June 4, 2025 (the “Original Contract”), is by and between Franklin
County, PA (The “County”) and South Central Community Action Programs, Inc. (The
“Contractor”). County and Contractor may be individually referred to as “Party” or collectively
as the “Parties” in recognition of the following:
WHEREAS, the Parties entered into the Original Contract whereby Contractor provides
coordination of behavioral health outpatient services; and
WHEREAS, the Parties wish to modify the contract.
NOW, THEREFORE, in consideration of the mutual promises contained herein and
other good and valuable consideration the receipt and sufficiency of which is expressly
acknowledged, the Parties agree to the following changes to the Original Contract:
1. Correction to the Emergency Solutions Budget amounts listed on page 17, Appendix A to
be:
RAPID REHOUSING $13,972.59
EMERGENCY SHELTER $96,000.00
HMIS $5,201.21
ADMIN $3,455.49
TOTAL BUDGET $118,629.29
2. All other terms and conditions of the Original Contract, except as expressly amended
herein, shall remain in full force and effect. To the extent this Amendment conflicts with
the Original Contract, the language of this Amendment shall be binding.

Amendment to be effective as of _____________________.
