HomeMy WebLinkAboutID Provider Meeting Minutes 5-3-13Franklin/Fulton Intellectual Disabilities Program Provider Meeting May 3, 2013 Minutes Welcome & Introductions Everyone introduced themselves and their agency affiliation. Fiscal Updates  Friendly Reminders for our Based Funded providers: - May 15th: Quarterly Expenditure Report - July 10th: Audit engagement/extension letter - July 31st: Progress reports on outcomes  Ed: Required Reports - Ed created a procedure statement to go along with the spreadsheet that Providers use to track assets - Procedure Statement MHIDEI-2013-418 – Tracking of Assets and Purchase with MH/ID Funds. There are three forms included in this pack – real estate purchases, real estate lease and fixed assets purchased. The tracking of assets is an annual report at the end of the fiscal year that is due August 15th. Ed explained that the report will recap all assets that providers purchased in whole or part with MH/ID funds. All procedure statements are on the county website. If anyone has any questions, please call or email Ed Franchi. - As a result of the Block Grant, the reporting of the Block Grant is requiring us to track Franklin County versus Fulton County. If a provider delivers services in both counties, the invoice sent to the County will need to be broken out between the two counties. The Block Grant technically started this fiscal year but the procedure statement will not go into effect until July 1st of this year. If you have any questions, please contact Ed, Jane or Marion. SCO  ISP Checklist - Matt Yingling and Laura Camacho from SAM met with Marion and Jane regarding the ISP Checklist. After reviewing the checklist, SAM thought the best place for providers to start is to look at training. The supports coordinators have received training. The way the process should work is that the checklist should be initiated from providers regarding information that is needed for the ISPs to be completed by the SCs. Providers should complete pages 1, 2 and 9 of the ISP Checklist. They should also attach any additional/updated information. It should be compiled in the form of a team meeting. Questions/Comments/Concerns: o Q: So the provider will only initiate the 6 month review? A: For the annual and critical revision, the SC will initiate. The providers will then be asked for certain information. o Marion asked providers what they are experiencing with the bi-annual reviews. Are there miscommunications, what are the issues?  Deb Thomas from OSI: we are only doing certain pages of the ISP Checklist. The program specialists at OSI are only doing the 1st page and the 9th page so checklists have been incomplete.  Tony Fisher from FCS: was confused on the start date for providers. The new checklist started for any annual plan after April 1st. The earliest 6 month would be in October. Tony thought that April 1st was the start of the 6 month due date.  Jim Schu from SAM: SAM is looking at it as being proactive. It can be interpreted beginning April 1st. They need to start being done. o Q: What is the expectation for the term end date and the date of the ISP? Does it need to be 30 days prior and does it go to the SC? A: Yes, it should go to the SC and it should be discussed at the team meeting. It is time sensitive and should be done 30 days prior. Providers are to be completing certain sections (page 1, 2 and 9) before the meeting so that the information can be put into the ISP. o If you are completing the ISP Checklist electronically, save your hard copy. Providers should maintain a copy of what they completed as well as what the SC completed. The final checklist is submitted for review. The AE submits it and returns the final copy back to the SC. o The bi-annuals are a good opportunity to look at service utilization. Marion has not seen any unit adjustments in the recent bi-annual reviews. This is a good opportunity to review the need for additional or reduction in units. . Lori Young  AE QM Plan - The quality management plan will be changing July 1st. Lori provided a draft copy that will also be sent out electronically. QI Council will be meeting later this month. The QM plan goes into effect before the next ID Provider Meeting. Since the document Lori provided was only draft, there is a chance it could change. SAM has input for the plan before it is implemented as well.  IM4Q - For those who have individuals surveyed, there are only 7 left. Lori attended the IM4Q Regional Meeting this week. Currently 1,400 surveys are completed. They have decided starting next year that number will be reduced to 400 surveys for the entire state. We will find out sometime in May how many of the NCI surveys Franklin and Fulton counties will have. The surveys will be distributed per population. Right now, Franklin/Fulton County does 30 and they are expecting that number to drop to less than 10 surveys.  WRAP Training - WRAP stands for Wellness Recovery Action Plan. Franklin/Fulton County along with Philadelphia have partnered with Copeland Center to hold WRAP training for individuals who have both an Intellectual Disability and a Mental Illness. The first training will be taking place in June. A second training will be held in September. An email with all the information will be sent to providers from Cori Seilhamer. Only one person per provider can be sent to the training due to limited slots. It is two day training in June and both the June and September trainings must be attended. ID and MH are partnering to hold this training. The workbook, WRAP for a Self- Determined Life will be published in December. Jane Cline  E-mail Addresses - County email addresses have changed. All county employees’ emails are now @franklincountypa.gov. A list will be sent out of the new email addresses.  AE Meeting Information: Initiatives - Lori and Jane attended the AE Meeting this past Wednesday. They learned the initiative that ODP started with Aging Care Giver. They are hoping to continue into next year into the Governor’s budget and have the wording changed so that we can support anyone who is on our emergency puns. They are also planning on doing the Graduate Initiative. We should be receiving more slots for graduates. There are currently 14 graduates this year. - The topic of an individual going into a nursing home for short term was discussed. That individual has to be put into “intent to enroll”. We cannot put them in consolidated. That waiver slot has to be held. This could be an issue for residential providers. The slot has to be held for 180 days. If it is more than 180 days and we feel that individual is not coming out of the nursing home, we can determine it is permanent and un-enroll them. - The discussion of Managed Care was brought up. As for right now, it does not look like they will move ahead with Managed Care in ID but it is not off the table. There will be a training held at the end of June regarding Managed Care. A trainer from Ohio will be coming in but it is only open for AEs. - Supplemental Habilitation has a new manual coming out. The announcement came out last week. They are doing away with the old checklist and we are using the ISP Checklist. Marion Rowe  IM: Incident deletion, Investigation Determination, IM analysis - Marion made copies of new updates with incident management, a hand out was provided. At the last provider meeting, we discussed the new requirements for Certified Investigation Summary. Marion wanted everyone to have information on how to delete an incident (Informational memo 110-12). You can just send an email with the individual’s information to have the incident deleted. Informational Memo 024-13 provides definitions for investigation determination. It also discusses if you are doing an investigation and you realize there is another allegation, you are to keep the two incidents separate. Overall for our county, incidents have improved greatly. We have stayed on top of finalizing them. Marion is still sending the monthly checks on what is still pending and what is overdue. You have 30 days to finalize your incident upon entering it. If you do not have all the information, you have to remember to request an extension. There are a lot of old incidents (some prior to 2006) that are still not finalized. Providers need to call the help desk or contact Theresa Toombs at ODP on how to get those cleared up. IM Analysis is due on June 14th. If you are not doing it locally, whoever your contact is in your agency, have them send it to Marion by the due date.  Transitioning Students - At the last provider meeting we were hoping to get waiver slots for our students who are graduating in June. We will not get our slots until the budget is passed. It is very likely after graduation that students will not have waiver funding. It is a good idea for SCs to be meeting with the families to look at other resources for the individuals until waiver funding is available. With the assistance of case management and our meetings, we will figure out what students we will be able to serve with base funding until waiver funding is available. If we are unable to meet their needs, we will look at other resources. HCQU – Deb Berg  Deb discussed a movie that was presented at Patton. It is an hour and half documentary. Deb explained that the movie was an eye opener regarding individuals who are noncompliant, abusive, violent, etc. She would like to show the movie but is not sure of the interest people would have. Deb will send an email out to see if anyone is interested. Please respond honestly and how many people would be coming. The movie could be counted as training.  There were 97 HCQU trainings in April and 800 people attended (this number is not just for Franklin county). The evaluations that attendees are filling out are coming back very positive. The Legal Seminar that was discussed at the last provider meeting was held. There was training on hoarding for the PPC meeting. If there are individuals with behavioral problems, please contact Deb or Marion so that case can be brought up at the PPC meetings.  There is now a consultant psychiatrist and a consultant pharmacist. This provides for individuals to receive a second opinion. For example: if an individual is concerned about medications they are on. There is a referral process. A nurse will go out to the individual’s house to do a CDC and they will gather information for the psychiatrist or pharmacist. The pharmacist is taking a week to give a report back and the psychiatrist is taking about a month. Any questions or referrals email or call Deb or Marion. Round Table Discussion  MHA has their 12th Annual Walk the Walk event on May 24th.  ARC passed out flyers regarding events for May and June.  OSI’s banquet is next Thursday, May 9th.  NHS recently had a Family Living Licensing. Licensing staff are saying based on the changes, everyone should be prepared a month or two earlier.  NHS has a new Behavior Support Specialist – Mike Wolkoski.  Bonnie Miller of New Light has a new home up and running. Things are going well.
