MEETING MINUTES
Welcome & Introductions
Everyone introduced themselves and their agency affiliation.
Fiscal Updates
» Friendly Reminders to our Base Funded Providers
August 15th ~ Quarterly Expenditure Report (April – June)
For FY ending 6/30: Salary review, Property Purchased, Property Leased,
and Fixed Assets Purchased
September 30th ~ MCR Report (program funded only)
15th of each month ~ monthly invoices, debarment and exclusion checks
If anyone has any questions in regards to FY 16-17 contracts, please contact Marion
(mrowe@franklincountypa.gov) and/or Jane (jcline@franklincountypa.gov).
Amendments for FY 15-16 have been completed.
Please check service authorizations as well to make sure there are no issues with
units that have been authorized. If you see any issues contact the Supports
Coordinator (SC) to make revisions to submit to Marion or Jane for approval.

» Autism Waiver Eligibility
Steve is part of a Waiting List Management Initiative that includes Office of
Developmental Programs (ODP), Supports Coordination Organization (SCO),
representatives, providers, Administrative Entities, etc. The initiative is currently on
draft #5. The goal is that autism is going to be included in the ID program eligibility
and waiver effective July 1, 2017. Eligibility requirements are Intermediate Care
Facilities for Individuals with Intellectual Disabilities (ICF/ID) and will now add
Intermediate Care for Individuals with Other Related Conditions (ICF/ORC). If an
individual has that diagnosis and significant limitations in at least three (3) areas,
they will be eligible for the ID program and waivers. This initiative will not do away
with the autism waiver, combine the autism waiver with consolidated or
person/family directed supports (PFDS) waivers nor add additional waiver slots.
There is consideration for replacing Prioritization of Urgency of Need for Services
(PUNS) with Risk Needs Assessments. Once an individual is deemed eligible for
waiver, a Supports Intensity Scale (SIS) will need to be completed at that time not
when services are received. LifeCourse planning will be added to the initiative. New
curriculum is being developed to train providers to support individuals who have
autism. At this point, the initiative is being looked at by the Deputy Secretary.
» Communities of Practice – Regional Collaborative
ODP has hired Nancy Ritchy. Nancy has presented at meetings across the state, such
as Administrative Entity (AE) Meeting and Independent Monitoring for Quality

(IM4Q) Meeting. She will be presenting at the Everyday Lives Conference on
developing LifeCourse Planning which is still in development. There has been
discussion between LifeCourse Planning and Regional Collaboratives with a
community navigator. There are 74% of individuals with intellectual disabilities
living in family homes. The LifeCourse Planning is geared for families to find natural
and generic supports to help support the individual and the family. Franklin/Fulton
County is looking into becoming part of the regional collaborative. A decision of
interest is required by the end of August. After that, a plan must be developed and
submitted on how it will be done. Currently, it is unknown how it will be funded.
Lori encourages providers to attend the Communities of Practice trainings in the
next months as it will be part of eligibility and service curriculum. The example
given at the Communities of Practice training is a three legged stool representing
discovery and navigation, connecting and networking and day to day services.

» Incident Management: Medical Related Incident Review & Closure
Incident Management Training is scheduled for Tuesday, August 16th from 10:00
a.m. to 12:00 p.m. at the Administrative Annex. The training is in regards to getting
medical related incident reports closed by ODP.
Concerns from providers entering incident reports:
The system is not user friendly. More and more medical terminology is being
expected by individuals who are not licensed medical personnel. Providers cannot
understand how ODP expects medical knowledge from staff that is not trained in
that manner. Providers continued to provide examples of issues they have had with
incidents.
Marion links incident reports together that she feels are related; providers have this
capability as well. If a provider feels that particular reports should not be linked
together, the link can be removed. Linking reports makes it easier for review and
alleviates the need to state “see Incident #” in the report.
There are no alerts when there is a comment to an initial report. Before completing
the final section, providers should click on regional initial review and county initial
review for comments. Incident reports have been bounced back due to providers
not being aware of comments and finalizing the report without including the correct
information.
There have been issues with incidents not getting submitted within the appropriate
timeframe. There is now an extra step involved in EIM; please make sure to
complete the proper steps to submit incidents. If providers find that they forgot to
submit an incident, go back into the report and insert a note “forgot to hit submit” as
ODP will ask why the report was late.

» UCP Pathways Update
The state should be completing their licensing for Pathways on August 8th or 9th
with an official opening being Wednesday, August 10th. An email will be sent out in
regards to opening. Staff has been in place for over a month now and the building is
open. If anyone is interested in a tour, please stop by. Pathways is located in
Chamber Hill Professional Center, corner of Norland and Fifth Avenue, right next to
the AM Vets. St. Paul’s Methodist Church is on the hill behind Pathways. An official
open house will take place at a later date. Invitations will be sent out.
Pathways falls under 2380 regulation for adult training facility. It is different from
the typical adult day program as it follows a set curriculum. It is a two (2) year
program with outcomes being competitive employment or independent living.
Currently, there are nine (9) applicants but Pathways can take up to a maximum of
twelve (12). The goal is for individuals to attend three (3) days a week to get the full
benefits of the curriculum and program. However, this is not a requirement. Hours
of operation are Monday through Friday, 9:00 a.m. to 3:00 p.m. For transportation
purposes, arrangements can be made before and after those hours for pick up and
drop off if needed as staff will be there.
The age limit is eighteen (18) years and older. If there is an older individual who is
interested in working in the curriculum they can be considered. There is no cap to
the maximum age. Part of the program is independent living and job skills. There are
lessons on healthy life styles, budgeting, shopping, cooking, etc.
» HCQU Updates: Residential Criteria/Assessment
At the last PPC Regional Meeting there was conversation on individuals who are
transitioning from RTF settings, state centers, jails, etc. and needing residential
services. The requirements and assessments prior to going into residential were
discussed. A lot of individuals are not interested in residential nor do they do well in
residential. However, the expectation is that group home placement is where
everybody should be. The AEs are trying to gather information from residential and
lifesharing providers on any assessments or criteria they look for prior to admission
to determine if licensed residential is the appropriate placement. If criteria is
provided it can be reviewed to see if the individual meets the level of care for
placement and if it is something they want. Currently, the only thing AEs have is the
residential checklist for the ISP. This is a document that provides information in the
ISP but does not state the criteria that must be met for the service. If providers have
any information please send it to Marion so she can share it at the next Regional PPC
Meeting.
The idea is to be in the least restrictive setting. Group home should be the last choice
as it is the most restrictive setting. Lifesharing is a requirement at the minimum
before anyone goes into a group home. Providers shared some of their criteria.

one criterion to look for is safety. Another is vacancies. Providers ask for as much
information as they can get. ISPs and psychological evaluations are helpful. Provider
discussed using the Assessment and Function Living Skills (AFLS). It scores
individuals on level of functioning on board range of areas. It is not used to say an
individual cannot be served but to see where the individual can progress.
In regards to group homes, Jane discussed that finding available housing other than
group homes is a struggle. The ID Program has opened four (4) additional
independent living apartments. The County holds the rental agreement but the
individual signs the lease with the landlord. The County helps supplement the rent.
The locations are convenient to stores, restaurants, pharmacies, etc. These
apartments have staff come into the home between the hours of 3:00 p.m. and 9:00
p.m. to do home and community and companion services. There are now ten (10)
independent living apartments through the ID Program.

» Everyday Lives Conference (handout)
My life, My way will be presented at the Everyday Lives Conference on September
14th – 16th. Pre-conference is September 13th at the Hershey Lodge. Registration
deadline for the conference is September 1st. Communities of Practice is a break out
session. New Everyday Lives book has been published and will be distributed at the
conference. The biggest changes to the book are adding My life, My way which is
written from self-advocates and what families value will be added.
» 6100 Regulations and Waiver Renewal
Waiver renewals are up as of July 1, 2017. The expectation of ODP is that the 6100
will also be approved at that time. The 6100 regulations will be out in October for
public comment and the Waiver Renewal will be available for public comment in
December. If anyone has any comments, send them to the mailbox that will be
provided. The Final Rule is now called the Rule. The expectation is to encompass the
Rule into the Waiver Renewals also.
Round Table/Provider Updates
» PA Mentor ~ Kathy Ibrahim
PA Mentor is restructuring; going to be separated by service lines. It is unknown
how this is going to work. Kathy will keep everyone informed as they learn more.
» Family Care Services ~ Nathan Sullivan
FCS has expanded capacity for Lifesharing. There have been ten (10) homes recently
opened who can accept placement if the individual meets criteria. If anyone has
anyone on their case load that may be interested in pursuing this, Nathan is more
than welcome to meet with them.

The ARC Social is August 18th at 9:00 a.m. at St. Paul’s Church. Kindly K9 Therapy
dogs will be there.
The ARC expanded their transportation to include Pathways and Red Tomato Farm.
If there is a high need, a wait list can be started.
The ARC recently became home owners as they purchased the building their office is
currently located in.

PDS has one individual looking for future placement options.
» Service Access & Management ~ Matt Yingling
Shanna Golden, ID Supervisor will be out of the office for three (3) months effective
sometime in August. Supports Coordinator Becky Frankovich will be out as well.
» Occupational Services, Inc. ~ Deb Thomas
OSI’s Annual Golf Tournament is September 29th. There are still openings for teams.
Annual Picnic is September 9th. It has been relocated to Fayetteville Fire Hall instead
of the Memorial Park.
