HomeMy WebLinkAboutMH Provider Meeting Minutes 12-9-16Mental Health Provider Meeting Human Services Building Friday, December 9, 2016 Page 1 of 6 MEETING MINUTES  Welcome and Introductions Everyone introduced themselves and their agency affiliation.  Provider Announcements and Updates » Family Care Services Sherri Holmes is the new Foster Care and Community Residential Rehabilitation (CRR) Program Director. » Summit Behavioral Health Services Due to the transcription department moving to home based work, outpatient services will be expanding space into the office next door. Summit will be recruiting for full time provider psychiatry, Licensed Clinical Social Worker (LCSW) and Licensed Professional Counselor (LPC). The recruiting process is taking place now but the position will not be filled until spring. » Mental Health Association The Leadership Academy is starting February 6th from 9:30 a.m. to noon at the MHA Office (478 Grant Street). The next CSP Meeting will take place December 19th from 10:30 a.m. to noon at the Franklin County Annex. Events Committee Meetings have been set up as well. If anyone is interested in being part of any of these, please contact MHA at (717) 264-4301. » Healthy Communities Partnership Tobacco Cessation classes are coming up. Classes start January 3rd at the Chambersburg Hospital at 6:30 p.m., January 23rd at Healthy Communities Partnership at 6:30 p.m. and starting in March on Tuesdays in Waynesboro. If you know of anyone who is interested, please contact HCP at (717) 264-1470. Operation Save a Life Training will take place January 12th at 6:00 p.m. at Trinity Lutheran Church. This training is on Naloxone, a medication that reverses opioid overdose. One dose of Naloxone is provided at the training for individuals eighteen (18) years and older. These trainings are held about twice a month. Strengthening Families is a new program for ages ten (10) – fourteen (14). It is for parents and children to learn skills. It is open to anyone interested not just specific target populations. The next class begins in January in Chambersburg. HCP is looking to start class in February in Waynesboro; once a location is Mental Health Provider Meeting Page 2 of 6 found the dates will be announced. There is 100% retention in current round of class. There is a possibility for another Facilitator training at a later date. » Occupational Services, Inc. OSI will be celebrating their 60th anniversary in 2017. Activities will be held such as Chamber Mixer and other special events. More information will come at a later date. » Mom’s Meals Jim attended a conference in Baltimore where two vendors provided in home meals, one being Mom’s Meals. He then had a conference call to obtain more information. Mom’s Meals is available to individuals on short term needing rehabilitation then return home. It is for individuals who have a challenge in regards to meal preparations who do not have others who can provide that. The meals can be delivered to the homes. Franklin County Area Agency on Aging works with Mom’s Meals. If you know of anyone who could benefit from this service, please contact 1-866-204-6111 or visit www.MomsMeals.com.  Rabbit Transit – Overview of Services Franklin County Transportation was outsourced to Rabbit Transit in April 2016. Rose Cook provided a history of Rabbit Transit; which has grown quite a bit even in the last year and a half starting with Franklin County. Effective December 1st, Perry County has now joined services. Due to serving nine (9) counties in central and south central Pennsylvania, Rabbit Transit’s official name is Central Pennsylvania Transportation Authority. Rose discussed the following information in regards to services provided: » Any individual who needs an escort, aid or assistant to go with them on a trip is able to as long as a form is filled out in advance; that person then rides for free. If there is a language barrier and the parent must attend as the guardian, if known in advance the rider may be able to take two people. If home care needs to go along with the guardian, it is preferred this is in writing from a doctor but then two would also be able to attend the trip. » There have been barriers to individuals having transportation due to not being able to take a child with the; it is now available that a parent can take one child along if it is known at the time of the reservation. » Ridership has increased 11% since Rabbit Transit has taken over services in Franklin County. Rabbit Transit is trying to do more outreach to let individuals know they are available to provide transportation. It is in hopes to be able to provide more services and meet more needs. » Rabbit Transit is striving to work hard on erasing the county lines and borders. There is a lot of opportunity to help more individuals and meet more needs by expanding the lines/borders. It allows for the sharing of resources. Currently, Mental Health Provider Meeting Page 3 of 6 with working among counties, it allows the opportunity to share drivers. There have been instances where a Franklin County driver has taken over routes for Cumberland County. » Maintenance has been enhanced to share resources and costs of parts and items ordered. Technology sharing has come a long way as well. There is a radio system that all employees are on the system but have their own channel. This allows for employees to talk among one another. York County is looking at CNG, an environmental process to reduce carbon footprint. Currently, York is the starting point but it is in hopes to expand this to other counties. It is felt the buses are safer due to enhancements in maintenance. There are 257 buses between all the counties. The safety department continues to grow. Rabbit Transit has a Rabbit Academy where trainings take place in York. It is individual training for proper ways to park, CDL licenses, driving piece, etc. and more emphasis on safety. » Rabbit Transit is curb to curb transportation. Drivers do assist clients to the door as long as the door can be seen from where the bus is stopped. » Some of the counties have fixed route; potentially, in the future, this will be looked into for Franklin County as it could benefit the community. » Everyone has to be preregistered for services. There are no same day services. An application must be submitted in order to start services. Those with medical assistance can request services for transportation then have thirty (30) days to get their application response. There is one application versus in the past where there were applications for MA, over 60, lottery program, disabled individuals, etc. Individuals will complete one application then when they reach each milestone, the changes are made behind the scene.  18 and up is persons with disability; medical assistance is any age with access card; MH/ID will authorize services; Aging is 60 and up and lottery is 65 and up. » There are different rules and regulations in each county. For Franklin County, the office for Aging has two different ways of paying for transportation; one is the individual pays 15% and the other they only pay a portion (waiver or gold card). Individuals who are 18 to 64 years of age pay 15% and Aging pays 85%. Rabbit Transit sells tickets. The tickets can be purchased online or through the mail. The individual then provides the bus with the ticket. » Find my Ride is a service available on the computer. Individuals can go on the website and book their own trip. This program is still in the works. » Fares are dictated by the County the person resides in. The Commissioners sign off on the authority of those fares and co-pays. This amount varies county to county. » There is currently a pilot program in York for veterans. It is a shuttle for veterans from York to Lebanon. This is a special grant that was received in York but it is hoped to expand this in the future. » Rabbit Transit does make trips to Hershey. The ride must be after 10:00 a.m. and come back at 1:00 p.m. A shuttle can be done in Hershey to pick up Mental Health Provider Meeting Page 4 of 6 individuals from Franklin, Adams and Cumberland counties. The route would be plotted out and each person would be picked up at their house. If someone has an appointment outside of those hours, fixed route in Carlisle can be tapped into. It is preferred that more than one day’s notice is given. » When an individual calls into the office to schedule a ride they are asked their pick up and drop off destination, doctor’s name, and additional reservation information. Between 5:00 p.m. and 7:00 p.m. the day before the reservation, the individual will receive a reminder call that states the reservation information and a half hour window of pick up time. » Mondays and Wednesdays peak call in times are in the morning. It is suggested to call in the afternoon. Staff is in the office from 7:00 a.m. to 7:00 p.m. Monday through Friday and 7:45 a.m. to 3:00 p.m. on Saturdays. There is a feature on the phone that if an individual does not want to hold and would like a call back they can provide their name and number. It is discouraged to do this for next day trips or cancellations; it is for more advanced notice rides. » Customer service has been increased to accommodate the increase of calls; 2,000 calls and 100 applications a day. Kim and Johanna Cordell from PerformCare are able to present “Accessing Children’s Mental Health Services” at staff meetings for any agencies that are interested. They can accommodate any group or setting. » TMCA Update – Genevieve Harper School based outpatient services is up and going. There have been five (5) meetings between providers and schools so far. It is off to a great start! Tuscarora Managed Care Alliance just received the ranking score card from IPRO; Franklin/Fulton is ranked #1 for 2015. This is in regards to thirty (30) day readmissions of psychiatric setting, thirty (30) day follow up, seven (7) day follow up, etc. Thank you to all providers! » PerformCare Update – Janina Kloster PerformCare is transitioning internal systems from Cabinet Share to Navinet. Navinet will be the main hub for processes that are electronic with PerformCare. A notice was sent out and is on the website. Cabinet Share will end effective close of business today. First thing Monday morning access to Mental Health Provider Meeting Page 5 of 6 Navinet will begin. Navinet is where requests and claims will be submitted. You can contact Janina at (717) 671-6551.  County Announcements/Information » Reports due since our last MH Provider Meeting  MCR Report (Program funded providers only)  Quarterly Report of Expenditures (July – September) » Reports due prior to our next MH Provider Meeting  Audited Reports (FY ending June 30th)  Progress Report on Outcomes in Appendix A  Internal Quality of Service Survey  Quarterly Report of Expenditures (October – December)  Appendix A for upcoming fiscal year  Budget/Rate Letter for upcoming fiscal year  Staff Roster for upcoming fiscal year (if appropriate)  Unit Assumptions for upcoming fiscal year (if appropriate)  Budget Narratives for upcoming fiscal year (if appropriate) » Psychiatric Admissions Reporting Overview (handout) This handout compares third quarter community psychiatric hospital admissions between calendars year 2015 and 2016. During the quarter, there was a significant decrease in total days, admissions and unduplicated persons. There was a slight increase in average length of stay. Holy Spirit Extended Acute Care had minor changes. The County is currently loaning two (2) of the three (3) beds allotted to them. There is a pending referral which may result in utilizing two (2) of our three (3) beds. At Danville, we remain six (6) over our bed cap of five (5) totaling in eleven (11) beds being occupied. There is some activity among individuals there. The reason for the overage in bed usage results in such reasons as having two (2) individuals who were at South Mountain Restoration Center who needed long term psychiatric care and another individual on forensic at Torrence State Hospital. In these particular situations, they must be admitted to Danville. » Sequential Intercept Model Discussion Our courts are possibly looking at Specialty Courts. With that research, the question arose in regards to a list of services that are available to refer individuals to at the different points of intercepts. We were able to provide this list for MH due to the Sequential Intercept Model. Drug and Alcohol also has a Mental Health Provider Meeting Page 6 of 6 similar list. If you have any updated programs or new services, please make sure to let us know so we can update our list. There are programs that are funded by forensics. We are trying to create a large visual to review it annually to make sure it is up-to-date. This will help identify gaps. The County is looking at the Co-Responder Model which is a mental health professional that works with law enforcement. It will be listed on there as a service that will be delivered to the community.  New Agenda Items » MH/ID/EI Administrator There will be a change in the MH/ID/EI Administrator position; this will not be happening immediately. The position will be posted in December. There is a process in the by-laws that must be followed. The target date for a new Administrator is March. Steve Nevada will continue to cover the department in the meantime; as well as continue to cover the Human Services Director position due to Rick Wynn’s retirement in October. » OMHSAS Primary Goals and Guiding Principles (handout) A conference call was held with Dennis Marion, Deputy Secretary of the Office of Mental Health and Substance Abuse Services that provided information on OMHSAS Guiding Principles and Primary Goals. Please review the handout provided during the Provider Meeting. Meetings are held quarterly at the Child Welfare Center in Mechanicsburg; attendance can be held in person as part of the Planning Committee.
