Provider Announcements and Updates » TrueNorth Wellness Services TrueNorth is finalizing the purchase of the Fulton County building that they currently provide services in. The goal is to officially own the building by August. » PA Counseling Services PCS Outpatient has purchased the M&T Bank building next to their current office in Chambersburg. There is no official move date yet as they are still waiting on permits to do some remodeling. The goal is towards the end of the year. PCS will let others know as they know. PCS has a new team leader – Rebecca Troutman. PCS will also be hiring new therapists. » Service Access & Management, Inc. Andrea Concordia introduced herself as the new Director of Operations for SAM Franklin/Fulton. SAM will be moving both Franklin and Fulton sites. Franklin’s site will not be done until September while Fulton’s site will be ready by July 1st. SAM is currently finalizing the lease for Fulton’s site. » Keystone Service Systems Keystone has announced that their Psychiatric Rehabilitation program will be closing effective August 2nd. Supporting Individuals Transitioning to Employment Success (SITES) – Michelle Lane (OSI) Over the course of ten (10) years, there has been a continual need for additional supports for individuals with Mental Health diagnosis to move towards competitive employment. There is a gap on what to do with programs and what staff can do. Occupational Services, Inc. (OSI) created a program that is nonpaid, non-work program that is geared towards targeting areas individuals need improvement/areas that are holding them back from employment (ex: transportation, managing coping skills, managing things going on to gain employment, etc.) called SITES. SITES is scheduled to start July 1, 2018. Amber Krenzer-Qartucci, Program Coordinator, has been developing curriculum for the program. The difference between SITES and other OSI programs – other programs need to be under Program Specialists and meet state licensing regulations. These programs also require the minimum time to attend the program is three (3) days a week. With SITES, if the individual is coming just for SITES and no other program, the minimum is four (4) hours a week. There is more flexibility to SITES. However, if the individual does decide to enroll in an additional OSI program then the three (3) day minimum applies. MH Provider Meeting MH 2 6/8/18 Something new with OSI – there is need for transportation. This year, OSI will look at centralized pick up points within a five (5) mile radius of the facility. When an individual comes into the SITES program, Amber will spend time getting to know them and identify their area(s) of need. Goals related to employment must be created (ex: work on attending regular program three (3) days a week). The goals should have a target date of no more than one (1) year initially. This does not mean that the individual cannot participate in the program for longer than a year though. The team will meet quarterly to review progress and see if goals need to be revised. This is a trial year for the program so there is a lot of flexibility. OSI is trying to figure out what works for individuals. Communication will be maintained with all team members including the SAM case manager. The referral process – individuals must go through SAM case management or OSI Program Specialist. SAM Case Managers have an OSI referral form. If the individual is not interested in other services, the referral will go directly to Michelle Lane. If the individual is interested in other OSI services, the referral will go to Deb Thomas. The maximum enrollment for SITES is 12 individuals but the maximum amount of individuals at one given time is six (6). This is a manageable number for staff as well as for the program area as it is not a huge area. OSI will have a waiting list for SITES as needed. The hours of operation are 8:00 a.m. to 3:15 p.m. SITES is for individuals 18 years of age and older. The program takes place at OSI at their Industrial Drive location. An area has been renovated for this program. The funding source for the program is Franklin/Fulton MH/IDD/EI. An open house will be held. Children’s Services » TMCA Update – Gen Harper The new initiative is for DLA-20 which is an outcomes measurement tool. This starts with outpatient then ends with inpatient facilities. Peer support will be towards the end of three (3) year limitation. This initiative is coming and will be discussed more when agencies start using it. A work group was put together for Youth Peer Support initiative for 14 years old and above that must be implemented by 2019. The work group met with providers. A RFP is out for current peer support providers to add the youth age range to their repertoire. Information is due next week. It is expected to have the service started by the end of summer (if providers have the staff hired to do so). » PerformCare Update No update at this time. » CASSP Update No update at this time. This memo pertains to the requirements of invoice submission due dates. If there are any issues with the date, please call Becky Leidig to work something out. » Psychiatric Admissions Reporting Overview Jim reviewed the Community Psychiatric Hospital Admissions Report for the first quarter of calendar years 2017 and 2018. There were 107 more bed days. The primary contributing factor was due to one individual being in the community hospital for 128 days then was admitted to Danville State Hospital. IDD (Intellectual & Developmental Disabilities) has been added to the report. These individuals are dually diagnosed. We have not been doing a good job at tracking this in the past but will start going forward. Danville State Hospital – one (1) individual was discharged on June 7th; dropping the total to 12 beds. Our bed cap is five (5). There is one (1) individual on the waiting list and another who will be referred from Torrance Forensic State Hospital. There was one (1) admission from Chambersburg Hospital to Danville. The individual was on the unit for 2 ½ months (as noted above). Due to the time it takes to get individuals into Danville, we are utilizing community hospitals as Extended Acute Care units (EACs) now. Update on EAC – there was potential with other counties for an EAC but at this point, it is not going to happen. Lancaster General, Ephrata Hospital and Philhaven all have EACs. We have expressed interest in them but so have other counties. The new norm is there is no EAC. As stated above, the community hospitals are now serving as our EAC. Individuals are going from community hospitals to Danville which is how it used to be years ago. MH Provider Meeting MH 4 6/8/18 » Sequential Intercept Model There is no update to the Sequential Intercept Model at this time. » New Agenda Items If your agency would like the opportunity to present during a provider meeting, please email Jim.
